It seems to be about abortion, to me (reminds me of The Sider House Rules, what with all the ether). But it's beautiful even if you don't interpret it any specific way.

One of the other videos on YouTube that talks about the album "White Chalk" says that PJ made a specific reference to this song as referring to a miscarriage. Well, they are called "natural abortions," which would make that technically accurate, but it is not about abortion so much as miscarriage, which would surely lead to a similar medicated state for treatment.

I'm not gonna argue with the songwriter.
However, the song itself does not give any clues towards a spontaneous abortion rather than an induced abortion. As far as I can tell it describes the experience of a medical procedure that is quite similar for induced abortions and incomplete spontaneous abortions (not all miscarriages require tr
eatment, but these do). Perhaps she left it ambiguous on purpose?
