You know, he/she might have "missed" on the first shot, and not died, and then finishing the job with a 2nd shot. I know a man that killed himself with a shotgun, using 2 rounds. This happened in his barn. 1st bullet blasted away half his face, then he went back in and reloaded the shotgun, which was the final, and deadly impact.
